  I give permission for photos and videos of my teen to be used for promotion and advertising through the following: Parish website, bulletin, and Youth program social media site; other parish-sponsored publications , and The People of God Magazine. I understand, as a parent I may request access to any public content that includes my teen. Note names will not be used.

  I have completed this application truthfully and to the best of my knowledge. By checking below, I understand the requirements and expectations of the program and registration process. Within three weeks of submitting this application, I agree to also submit all additional required paperwork {Baptismal certificate and Confirmation Requirements Acknowledgements Form (CRAF)}. Failure to do so will result in this registration being canceled.
  Requirements:
  Active family registration at Prince of Peace Catholic Community or written permission from your home parish Pastor
  Two Years of spiritual preparation before receiving the Sacrament (Freshman and Sophomore)
  No more than 3 absences per program year
  1^st year must attend One retreat and Second Year Confirmandi need to attend 2 retreats
  (2nd year 1-weekend retreat off-site and 1-day Sponsor retreat)
  Regular attendance at Sunday Mass -Attend as a group during Conf. night Sun. 5 pm once a month as noted in the calendar
  $60 for program fees (registered POP parishioner- $70 Non-registered) (checks made payable to Prince of Peace
  or online giving with the receipt emailed to iarvizo@popabq.org)
  Complete all written assignments throughout the program on time
  A copy of Confirmandi’s Baptismal certificate
  A signed CRAF Form
